Countryside offers a comprehensive nutritional management program for residents and patients with nutritional disorders as well as those with weight management issues. Individualized plans are developed for both risk reduction and/or management of the medical conditions impacted by an individual’s nutritional status, including diabetes, cancer, cardiovascular disease, hypertensive disorders, lipid disorders, and gastrointestinal dysfunction. When focused on the management of obesity, services include nutritionist-designed meal planning, exercise, nutritional education, counseling and behavioral support needed to accomplish long-term weight management goals. For individuals who, for a variety of health reasons, cannot receive food and nutrients through their mouths, Countryside provides enteral (tube) nutrition among its extensive range of specialized care services.
