Prior to discharge, our rehabilitative team makes sure patients are ready for the challenges involved with returning home or to an alternate level of care.  This may involve a visit to the resident’s home to assess possible alterations and adjustments that will enhance safety, accessibility and convenience. In some cases, the therapists themselves will visit the patient’s home. Arrangements for wheelchair assistance or other assistive devices, dietary counseling, wound care and medication management are among the many other services available to short-term rehab patients.
